Provides comprehensive care management for individuals with chronic health conditions, including mental health needs by coordinating medical, behavioral health, and social services to ensure support and health outcomes.

Eligibility: 21 years or older, enrolled in Medicaid, resident of area served and have one or more of the following conditions: Significant mental illness or serious emotional disturbance HIV/AIDS Two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.)

Required Documentation: Call for information.

Fees: Medicaid.

Coverage Note: Monroe, Ontario, Livingston, Seneca, Yates and Wayne Counties.
