Naturally Occurring Care Networks (NOCN)

FLPPS is divided into five geographic sub-regions, termed Naturally Occurring Care Networks (NOCN). These Networks represent the full continuum of care and organizational leadership within a shared geographic service area. Each NOCN is led by a participant workgroup that represents the healthcare providers and community based organizations in their area.

Workforce Investment Organization for Western New York, WIO

In April 2018, FLPPS was designated a Workforce Investment Organization for Western New York focusing on the Long Term Care (LTC) Workforce. Commonly referred to as WIO (Wee-OH), we are proud to be the only PPS with this designation. The purpose of this work is to reduce avoidable hospitalizations, increase community-based care, and enhance the skillsets of the long-term care workforce. As part of this program, we are partnering with two Managed Long Term Care Organizations; Nascentia Health and VNS Choice. Through this partnership, we aim to develop solutions that support training, recruitment, and retention of the LTC workforce.

Goodwill of the Finger Lakes, 211 LIFE LINE

FLPPS is proud to partner with Goodwill of the Finger Lakes through their 211/LIFE LINE program to bring a specialized Healthcare Workforce resource directory to the Finger Lakes region. The directory is a comprehensive, informational resource to support individuals interested in entering and advancing within the long-term care workforce. To address the many recruitment and retention challenges our health care providers face, the directory is a shared space for our valuable community partners to showcase their training opportunities. The resource directory lists training and professional development opportunities by county, as well as many other employee support services including childcare, transportation, and pre-employment physicals.
