thru: Town of Gates/Town Hall Community Center
• Provides leisure activities and services for older adults.
• Offers medical transportation for appointments only within Monroe County. Limited to two rides per month.
• Provides transportation to senior club activities for residents of Dunn Tower and Gateway.
Eligibility: Ages 50 years and older and 2 medical appointments per month must be in Monroe County subject availability. Resident of area served.
Application Process: Call for information.
Required Documentation: Call for information.
Fees: No fee for services. Transportation: donations accepted but not required. Homebound counseling provided by Eldersource. Eldersource may charge a nominal fee.
