Provides self-directed services, including fiscal intermediary and broker.

Oversees residential houses providing support to persons for medical appointments, meal prep, socialization, budgeting, and daily living skills.

Provides Individual Support Services (ISS) housing/rent subsidies for persons with I/DD.

Eligibility: Diagnosis of intellectual/developmental disability (I/DD).

Application Process: Call for information.

Required Documentation: Documented diagnosis of Intellectual Developmental Disability before age 22 years. Medicaid eligible.

Fees: No fee for services.

Coverage Note: Cayuga, Livingston, Monroe, Ontario, Orleans, Seneca, Yates and Wayne Counties.

Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff

Languages: Spanish- Staff provides in-person interpreter, Materials/literature in language indicated, Website can be translated
