• Provides self-directed services, including fiscal intermediary and broker.
• Oversees residential houses providing support to persons for medical appointments, meal prep, socialization, budgeting, and daily living skills.
• Provides Individual Support Services (ISS) housing/rent subsidies for persons with I/DD.

Eligibility: Diagnosis of intellectual/developmental disability (I/DD).
Application Process: Call for information.
Required Documentation: Documented diagnosis of Intellectual Developmental Disability before age 22 years. Medicaid eligible.
Fees: No fee for services.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Orleans, Seneca, Yates and Wayne Counties.
Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff
Languages: Spanish- Staff provides in-person interpreter, Materials/literature in language indicated, Website can be translated
