Provides a comprehensive outpatient substance abuse treatment program. Services in person and via Telehealth.
  • Evaluation and assessment. 
  • Outpatient treatment. 
  • Individual, Group and Family Counseling. 
  • Treatment and rehabilitation of minority and low income individuals with substance abuse and addiction. 
Offers educational workshops and presentations. 

Offers a Twelve step program.
  • Groups and individual counseling. 
  • Available in English and Spanish. 
  • Deals with complex inter-related problems associated with poverty addiction, behaviors and social determinants of health. 
Offers Relapse Prevention groups and services. 

Offers Medication Assisted Treatment (Suboxone) for Opiate Addiction.

Eligibility: Resident of area served, Substance abuser, ages 17 or older.

Application Process: Call for information or appointment.

Required Documentation: Proof of ID and insurance card.

Fees: Medicaid, Medicare, Blue Choice Option, Fidelis, MVP, Sliding fee scale.

Coverage Note: Monroe and Ontario Counties.
