Eligibility: Birth to age twenty one. Medicaid eligible.

Application Process: A referral can be made through by email.

Required Documentation: Medicaid ID.

Fees: No fee for services.

Coverage Note: Albany, Cayuga, Chemung, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Monroe, Montgomery, Niagara, Oneida, Onondaga, Ontario, Oswego, Schenectady, Seneca, Steuben, Tompkins, Wayne and Yates Counties.
