Eligibility: Adults age 18 and over. Adults do not have to be referred from an agency or provider, self-referrals are accepted for all treatment programs. Adolescents age 13 and over by referral only.

Required Documentation: Health insurance card.
Fees: All major insurance coverage including Medicaid and Medicare and managed care coverage, self-pay and a sliding fee scale for those who do not have health insurance coverage.
Coverage Note: Monroe, Livingston, Cayuga, Ontario, Seneca, and Wayne counties.
Languages: ASL. Translation services available for other languages.
