Community Based Peer Support Services for Youth and Families
thru: Mental Health Association of Rochester/Monroe County
• Provides formal and informal services and support to youth, who are experiencing social, medical, emotional, developmental, substance use, and/or behavioral challenges at home, school, placement, and/or community-centered services.
Language Access
Eligibility: Youth ages 13 to 20 years old with emotional, behavioral and medical health challenges and their families.
Application Process: Must have referral. Visit website for program specific referral information.
Required Documentation: Referral medical necessity, youth programs Medicaid managed care.
Fees: No fee for services.
Coverage Note: Monroe County.
