Care Management: M–F: 9:00AM. to 5:00PM.
* Provides care management and support services including:
- Meeting personal health goals and outcomes
- Coordinate transportation to medical visits
- Understanding and providing medical needs including specialized care
- Provide insurance assistance (ACA Marketplace, ADAP, private insurance, Medicaid and Medicaid spend-down, Medicare Part D)
- Attend medical appointments and pharmacy visits for medication follow-up
- Find primary care and other kinds of medical care
- Coordinate referrals and follow through for mental health and substance abuse
- Explore other services, like nutrition education, day health, our food pantry, and more
- Housing assistance and planning
- LGBTQ-focused Care Management services

Fees: Case management: Medicaid billed or no charge. Other services: no fee for services.
