* Provides diagnostic evaluation services for individuals with disabilities that impact their finding or keeping employment.
  • Medical evaluation.
  • Psychological evaluation.
  • Rehabilitation.
* Provides vocational counseling.

* Offers vocational training.

* Offers job placement services.

 

Eligibility: Individuals with disabilities that interfere with employment who require vocational rehabilitation. High school students who are transitioning to adult services are also served.

Application Process: Referrals through doctor, social worker, agencies, or self. Call for orientation dates and times.

Required Documentation: Documentation of disability; Social Security Disability or Supplemental Security Income documentation if applicable.

Fees: Counseling, testing, placement: none. Medical services and some training: sliding scale based on income.

Coverage Note: Monroe, Livingston, Ontario, Seneca, Wayne, and Yates counties.
