Eligibility: Primarily Livingston County residents but also provides services to residents of surrounding counties.
Application Process: Intakes are done by appointment only.
Required Documentation: None.
Fees: Medicaid/Medicare. Third Party Insurance. Sliding scale based on income. No one denied if unable to pay.
Coverage Note: Primarily Livingston County and some surrounding counties(Monroe, Steuben, Wyoming and Genesee).
Languages: American Sign Language, Spanish- Language line
