Provides a residential health care facility, providing long-term care for individuals with complex health conditions.

Provides long-term care for pediatric and young adults with congenital defects, chronic illness or head/spinal cord trauma.

Provides a specialized unit for individuals with dementia, wandering behavior, and Alzheimer's disease..

Offers a short-term rehabilitation program, providing individualized care and treatment to people who have disabling conditions as a result of an injury or illness. Treatment is provided by a team of specialists.

Provides a newly established Hospice/Palliative Unit for those requiring end-of-life care.

Volunteer opportunities through the Friendly Visitor and Transporter Volunteer Programs.

  • Will provide training
  • Friendly Visiting requires 10 volunteers, Transporter Volunteer requires 5 volunteers
  • Friendly Visiting 2-4 hours per shift. Transporter Volunteer 1.5-3 hours per shift
  • Can be used to fulfill ABAWD (Able-bodied without dependents) hours

Eligibility: Varies according to program. Call for information. Volunteer Programs: People skills, desire to support others, and ability to walk long distances while pushing a wheelchair through the facility.

Application Process: Referrals through physician, health facility, agency or family member. Friendly Visiting Program: Contact the volunteer program by phone, schedule and in-person meeting.

Required Documentation: Call for information. Friendly Visiting Program: Background check, NYS ID, health history form, immunization records, and complete orientation packet.

Fees: Medicare. Medicaid. Third-party insurance. Friendly Visiting Program: No fee for services.

Coverage Note: Genesee, Livingston, Monroe, Ontario and Wayne counties.
