Eligibility: Male 18 years or older. Post inpatient discharge and unhoused.
Application Process: Referral from previous inpatient program.
Required Documentation: Photo Id, Insurance card.
Fees: Medicaid.
Coverage Note: Greater Rochester area.
Languages: Spanish, American Sign Language- Staff provides in-person interpreter
